Headstone marked by an obelisk on a three-tier plinth and two-tier base. Carved cross on the obelisk, below in a medallion a relief bust of Christ wearing a crown of thorns. The first step of the base with chamfered upper edges, glazed. Second step of pedestal with moulded upper edges, engraved inscription to front (2). The third step of the pedestal with a moulded wider plate at the top, on the front an engraved inscription (1). The gravestone enclosed by a cast-iron fence, consisting of bars terminating in a pyramid.

Inwentaryzacja cmentarzy dawnego powiatu zbaraskiego rozpoczęta została w roku 2019 przez zespół z Instytutu Historii Sztuki Uniwersytetu Kardynała Stefana Wyszyńskiego w Warszawie pod kierunkiem prof. Anny Sylwii Czyż i dr. Bartłomieja Gutowskiego. Ze względu na pandemię COViD, a następnie działania wojenne prace od roku 2020 zostały wstrzymane. Materiał opracowany został w formie kart ewidencyjnych obiektów zabytkowych i w formie kart inwentaryzacyjnych w katalogu on-line - http://cmentarzetarnopolskie.uksw.edu.pl/. Dokumentacja obejmuje wszystkie nagrobki związane z polskim dziedzictwem kulturowym (przede wszystkim rozstrzygający jest napis w języku polskim) powstałe przed 1945 rokiem.
