Tombstone originally marked by a cross on a plinth framed by a concrete band in the form of a coffin lid. The cross is not preserved. Inscription on the front of the band. On the pedestal from the front a rectangular photographic glass framed by the letters Ś. P. decorated with floral ornaments, below an engraved olive branch inclined to the right, below an inscription. Inscriptions engraved (typeface printed).

Inscription:

Ś. P . / JÓZEF / ZAKRZEWSKI / + 16 LUT. 1918 R. LAT 56. //. WIE [...] EMU / pokó [...] jego / PAMIĘCI STROSKANA / ŻONA I DZIECI //.
