According to the file of Lucjan Uziębła, the gravestone took the form of a wooden cross, painted white, with a plaque bearing the inscription: Ś. + P. / Kacper / Piłsudski / Born 15 October 1879 / Died 28 July 1915. //. In Wacław Wejtka's card file, under the letter P, card number 173, dated 1936. According to the file, the location of the gravestone is the sector "At the back behind the chapel on the left side of the path". Next to the description of the gravestone a note: "wooden cross, inscription on a tin plate. Cemented grave without a fence". On the plate inscription: S + P / KACPER / PIŁSUDSKI / Ur: 15. X. 1879 R / DIED: 28. VII. 1915 R //.
