Burial place marked by a figural group on a two-stepped plinth and three-stepped base with two panels added. Figures of St Charles and Virgin Mary on their own plinth. Saint Charles dressed in liturgical garb and a short cloak fastened at the neck. In his hands he holds a cross. Short hair and pius on head and shod feet. The figure of the Virgin Mary dressed in a long gown, veil and mantle. Bare feet visible from under the gown. Hands folded in prayer. The first step of the plinth with profiled upper edges. An engraved inscription to the front. The second step of the pedestal finished with a simplified beam and covered with a wider panel. On the front a niche closed with a full overhanging arch. In it, a relief carved figure of Christ. He is dressed in a long robe and mantle. With his right hand he makes a gesture of blessing, his left hand at waist level. On his chest a burning heart. Plates with engraved crosses.

Inwentaryzacja cmentarzy dawnego powiatu zbaraskiego rozpoczęta została w roku 2019 przez zespół z Instytutu Historii Sztuki Uniwersytetu Kardynała Stefana Wyszyńskiego w Warszawie pod kierunkiem prof. Anny Sylwii Czyż i dr. Bartłomieja Gutowskiego. Ze względu na pandemię COViD, a następnie działania wojenne prace od roku 2020 zostały wstrzymane. Materiał opracowany został w formie kart ewidencyjnych obiektów zabytkowych i w formie kart inwentaryzacyjnych w katalogu on-line - http://cmentarzetarnopolskie.uksw.edu.pl/. Dokumentacja obejmuje wszystkie nagrobki związane z polskim dziedzictwem kulturowym (przede wszystkim rozstrzygający jest napis w języku polskim) powstałe przed 1945 rokiem.
