Headstone marked by a statue of the Virgin and Child on a two-stepped plinth and two-stepped base. The figure on its own base. Mary supports the Child with both hands. Dressed in long heavily ruffled robe and mantle, crown on head, long hair. Bare feet visible. Child's head perched. First step of plinth with chamfered top edges. Rectangular plaque to the front. Second step of plinth with simplified beam consisting of architrave and cornice, covered by wider panel. To the front an engraved inscription.

Inwentaryzacja cmentarzy dawnego powiatu zbaraskiego rozpoczęta została w roku 2019 przez zespół z Instytutu Historii Sztuki Uniwersytetu Kardynała Stefana Wyszyńskiego w Warszawie pod kierunkiem prof. Anny Sylwii Czyż i dr. Bartłomieja Gutowskiego. Ze względu na pandemię COViD, a następnie działania wojenne prace od roku 2020 zostały wstrzymane. Materiał opracowany został w formie kart ewidencyjnych obiektów zabytkowych i w formie kart inwentaryzacyjnych w katalogu on-line - http://cmentarzetarnopolskie.uksw.edu.pl/. Dokumentacja obejmuje wszystkie nagrobki związane z polskim dziedzictwem kulturowym (przede wszystkim rozstrzygający jest napis w języku polskim) powstałe przed 1945 rokiem.
