Headstone marked by a gravestone, at the head of which is a statue of the Virgin of the Apocalypse on a two-stepped plinth and two-stepped base. The figure on its own polygonal base. The Virgin with her head bowed, undulating robe, with her hands folded over her heart. She stands on a globe and tramples a serpent with her bare feet. First step of plinth with chamfered upper edges. Second step of pedestal with simplified beam, comprising architrave and cornice, inscription carved on front.

Inwentaryzacja cmentarzy dawnego powiatu zbaraskiego rozpoczęta została w roku 2019 przez zespół z Instytutu Historii Sztuki Uniwersytetu Kardynała Stefana Wyszyńskiego w Warszawie pod kierunkiem prof. Anny Sylwii Czyż i dr. Bartłomieja Gutowskiego. Ze względu na pandemię COViD, a następnie działania wojenne prace od roku 2020 zostały wstrzymane. Materiał opracowany został w formie kart ewidencyjnych obiektów zabytkowych i w formie kart inwentaryzacyjnych w katalogu on-line - http://cmentarzetarnopolskie.uksw.edu.pl/. Dokumentacja obejmuje wszystkie nagrobki związane z polskim dziedzictwem kulturowym (przede wszystkim rozstrzygający jest napis w języku polskim) powstałe przed 1945 rokiem.
