Headstone marked with cast iron cross on two-stage plinth and two-stage base. Cross on plinth. The arms of the cross capped with oak leaves. At the intersection of the arms a figure of Christ, above a titulus with the inscription: INRI. Plinth with figures of the Virgin Mary, St John and kneeling Mary Magdalene, below two putti. First step of plinth with chamfered upper edges. Second step of plinth with simplified beam consisting of architrave and cornice, covered by wider moulded panel. To the front, in a panel enclosed by an overhanging full arch, a carved figure of the Virgin of the Apocalypse. Mary holds her hands in a prayer gesture, standing on a globe. She is dressed in a long robe and cloak, fastened with a clasp. Long hair. Bare feet visible. Below, engraved inscription 1. To the right, engraved inscription 2.

Inwentaryzacja cmentarzy dawnego powiatu zbaraskiego rozpoczęta została w roku 2019 przez zespół z Instytutu Historii Sztuki Uniwersytetu Kardynała Stefana Wyszyńskiego w Warszawie pod kierunkiem prof. Anny Sylwii Czyż i dr. Bartłomieja Gutowskiego. Ze względu na pandemię COViD, a następnie działania wojenne prace od roku 2020 zostały wstrzymane. Materiał opracowany został w formie kart ewidencyjnych obiektów zabytkowych i w formie kart inwentaryzacyjnych w katalogu on-line - http://cmentarzetarnopolskie.uksw.edu.pl/. Dokumentacja obejmuje wszystkie nagrobki związane z polskim dziedzictwem kulturowym (przede wszystkim rozstrzygający jest napis w języku polskim) powstałe przed 1945 rokiem.
