Tombstone marked by a statue of St Francis with Child on a two-stage plinth and two-stage base. The figure on its own base. The saint supports the Child on his left hand and holds a flower branch in his right hand. Dressed in habit, tied at waist with cord, tonsure on head. Bare feet visible. The second step of the pedestal covered by a wider profile panel. On the front, in a panel enclosed by a full overhanging arch, a relief of the Virgin Mary. Mary with her arms crossed on her chest. Dressed in long ruffled robe and mantle. Long hair. Bare feet visible. Below engraved inscription 1. First step of plinth with chamfered upper edges. On the front, engraved inscription 2.

Inwentaryzacja cmentarzy dawnego powiatu zbaraskiego rozpoczęta została w roku 2019 przez zespół z Instytutu Historii Sztuki Uniwersytetu Kardynała Stefana Wyszyńskiego w Warszawie pod kierunkiem prof. Anny Sylwii Czyż i dr. Bartłomieja Gutowskiego. Ze względu na pandemię COViD, a następnie działania wojenne prace od roku 2020 zostały wstrzymane. Materiał opracowany został w formie kart ewidencyjnych obiektów zabytkowych i w formie kart inwentaryzacyjnych w katalogu on-line - http://cmentarzetarnopolskie.uksw.edu.pl/. Dokumentacja obejmuje wszystkie nagrobki związane z polskim dziedzictwem kulturowym (przede wszystkim rozstrzygający jest napis w języku polskim) powstałe przed 1945 rokiem.
