Tombstone marked by a statue of St Rozalia on a two-stage plinth and two-stage base. Statue on its own base. The saint is kneeling. Head not preserved. Probably secondarily set male head, with beard. Hands forfeited. By the right leg an open book, with skull. Sandals visible. First step of plinth with chamfered upper edges. First step of plinth with chamfered upper edges, covered by a wider panel. Engraved inscription to front.

Inwentaryzacja cmentarzy dawnego powiatu zbaraskiego rozpoczęta została w roku 2019 przez zespół z Instytutu Historii Sztuki Uniwersytetu Kardynała Stefana Wyszyńskiego w Warszawie pod kierunkiem prof. Anny Sylwii Czyż i dr. Bartłomieja Gutowskiego. Ze względu na pandemię COViD, a następnie działania wojenne prace od roku 2020 zostały wstrzymane. Materiał opracowany został w formie kart ewidencyjnych obiektów zabytkowych i w formie kart inwentaryzacyjnych w katalogu on-line - http://cmentarzetarnopolskie.uksw.edu.pl/. Dokumentacja obejmuje wszystkie nagrobki związane z polskim dziedzictwem kulturowym (przede wszystkim rozstrzygający jest napis w języku polskim) powstałe przed 1945 rokiem.
