Headstone marked by a gravestone, at the head of which is a figure of the Virgin Mary on a two-stepped plinth and two-stepped base. Mary with folded hands in a prayer gesture. Dressed in a long ruffled gown and cloak. She stands on a globe and tramples a serpent. Second step of base with chamfered upper edges. Second tier of pedestal with simplified beam, comprising architrave and cornice, covered by a wider panel, inscription carved on front.

Inwentaryzacja cmentarzy dawnego powiatu zbaraskiego rozpoczęta została w roku 2019 przez zespół z Instytutu Historii Sztuki Uniwersytetu Kardynała Stefana Wyszyńskiego w Warszawie pod kierunkiem prof. Anny Sylwii Czyż i dr. Bartłomieja Gutowskiego. Ze względu na pandemię COViD, a następnie działania wojenne prace od roku 2020 zostały wstrzymane. Materiał opracowany został w formie kart ewidencyjnych obiektów zabytkowych i w formie kart inwentaryzacyjnych w katalogu on-line - http://cmentarzetarnopolskie.uksw.edu.pl/. Dokumentacja obejmuje wszystkie nagrobki związane z polskim dziedzictwem kulturowym (przede wszystkim rozstrzygający jest napis w języku polskim) powstałe przed 1945 rokiem.
