Burial site marked by cross on plinth, banded as coffin lid. Cross and plinth perpetuated, lying side by side. Plinth with 1/3 chamfered sides and chamfered top edges. On the front a gabled closed plaque with an oval blank after a photograph, below an engraved inscription (1). On the band to the front an engraved inscription (2), below this an engraved cross and palm branches.

Inscription:

1. TU SPOCZYWA NASZ / [...] SYNEK / STAŚ / SZUMIKOWSKI / UR [.] 5 CZERWCA / 1915[R] ZM. 30 LISTOPADA / 1923 R. //.
2. [Ś]pij spokojnie / drogi Aniele! //.
