Burial place marked by a statue on a two-stage base and two-stage plinth. Statue of St Teresa in frontal, full-face view. The saint dressed in long, frilly nun's garb, long loose hair over which is a headscarf, hands folded on breasts, supporting a burning heart under which is a host. The figure on its own base. First and second steps of base piled up. First step of plinth chamfered at top, inscription engraved on front. The second step of the plinth surmounted by a cornice and simplified beam with a sigmoid gable to the front. Below, an arcaded panel in which is a relief figure of St Teresa in full-figure frontal view. The saint is dressed in nun's garb with a shawl on her head, holding a cross in her right hand against her breast in her left shawl.

Inwentaryzacja cmentarzy dawnego powiatu buczackiego wykonana została w latach 2021-2022 9 przez zespół specjalistów z Instytutu Historii Sztuki Uniwersytetu Kardynała Stefana Wyszyńskiego w Warszawie i Muzeum Narodowego w Warszawie w składzie dr Paweł Drabarczyk vel Grabarczyk, dr. Bartłomiej Gutowski (kierownik projektu), dr Magdalena Olszewska dr Magdalena Tarnowska. Materiał opracowany został w formie kart ewidencyjnych obiektów zabytkowych i opublikowany w katalogu Instyttutu Polonika. Dokumentacja obejmuje wszystkie nagrobki związane z polskim dziedzictwem kulturowym (przede wszystkim rozstrzygający jest napis w języku polskim) powstałe przed 1945 rokiem.
