Burial site originally marked by a cross and stele on a two-stage base. Cross perpetuated, lies nearby. Stele overturned, lies on base. On the front, in the upper part, a concave medallion in which a relief left profile of Christ wearing a crown of thorns. Below an oval panel in which a porcelain photograph, in which is a bust of a man, wearing a white shirt and dark jacket, supporting his head on his right hand. Below the photograph an engraved inscription. The slab originally framed on two sides by tracery, surmounted by a pyramidal form, under which a flower and a band are engraved on four sides. The lower part of the run-offs originally terminating in vases. The base is glazed.

Inwentaryzacja cmentarzy dawnego powiatu zbaraskiego rozpoczęta została w roku 2019 przez zespół z Instytutu Historii Sztuki Uniwersytetu Kardynała Stefana Wyszyńskiego w Warszawie pod kierunkiem prof. Anny Sylwii Czyż i dr. Bartłomieja Gutowskiego. Ze względu na pandemię COViD, a następnie działania wojenne prace od roku 2020 zostały wstrzymane. Materiał opracowany został w formie kart ewidencyjnych obiektów zabytkowych i w formie kart inwentaryzacyjnych w katalogu on-line - http://cmentarzetarnopolskie.uksw.edu.pl/. Dokumentacja obejmuje wszystkie nagrobki związane z polskim dziedzictwem kulturowym (przede wszystkim rozstrzygający jest napis w języku polskim) powstałe przed 1945 rokiem.
