The plaque commemorates the Polish sailors who died in the defence of Malta, especially those who fell on 16 June 1942 in the last phase of Operation Harpoon, when the destroyer escorting the convoy to Malta, ORP 'Kujawiak', sank after running into a mine. The plaque is located in Upper Barrakka Gardens in Valletta and was placed there on the anniversary of this tragic event, 16 June 2015.
