Burial place marked by a headstone on a band with two slightly convex slabs on it. At the head a plinth in the form of a wall. On it a stele and two tablets with engraved inscription. The stele is topped with a three-sided cross in relief.

Tu odpoczywa/ ś.p./ JERZY SMELIK/ właść. realności / * 18./6.1872 + 23./III. 1923 / żona jego ANNA/ * 23./2.1877 + 20./2.1942 //.
