Headstone marked by a gravestone, at the head of which is a statue of the Virgin Mary on a two-stepped plinth. The figure on its own base. Mary holds a book in her right hand, her left hand on her chest. She is dressed in a long ruffled robe and mantle, with a veil on her head. Shoes visible. First step of plinth with chamfered top edges. Engraved inscription to the front. Second step of plinth with simplified beam consisting of architrave and cornice, covered by wider panel. To the front, a panel enclosed by a full arch, with a relief carved baptism of St Martin (?).

Inwentaryzacja cmentarzy dawnego powiatu zbaraskiego rozpoczęta została w roku 2019 przez zespół z Instytutu Historii Sztuki Uniwersytetu Kardynała Stefana Wyszyńskiego w Warszawie pod kierunkiem prof. Anny Sylwii Czyż i dr. Bartłomieja Gutowskiego. Ze względu na pandemię COViD, a następnie działania wojenne prace od roku 2020 zostały wstrzymane. Materiał opracowany został w formie kart ewidencyjnych obiektów zabytkowych i w formie kart inwentaryzacyjnych w katalogu on-line - http://cmentarzetarnopolskie.uksw.edu.pl/. Dokumentacja obejmuje wszystkie nagrobki związane z polskim dziedzictwem kulturowym (przede wszystkim rozstrzygający jest napis w języku polskim) powstałe przed 1945 rokiem.
