Burial place marked by a cross on a plinth (broken cross resting on the left side of the plinth), set in a cast iron fence, the plinth in the form of a stylised hill, on its front smoothly polished - an inscription carved into the surface of the stone, in front of the plinth a concrete band.

Inscription:

Boże Twoja wola. / S. + P. / Helena Usas / 1876 - 1910. / Apolonia um. 1898.
