Burial site originally marked by cross on plinth, in band in form of coffin lid. Cross and plinth perpetuated, lying next to band. Cross on plinth. Plinth with 1/3 chamfered sides. Engraved inscription to the front.

FELIKS / ROMAN/ ZM. 14 XII 1918 R. / W WIEKU LAT 69 / [PO]KÓJ TWEJ DUSZY / [D]ROGI OJCZE //.
