An irregular plaque with a cross in a square panel at the front, an engraved inscription below, and a panel from a nonpreserved coat of arms.

Additional information:

Inscription lines 7 and 8 in italics. She was the second wife of Antoni Zabiełło, with whom she had a daughter Antonina, born on 22 February. According to Wacław Wejtka's records the plaque was located in the catacombs a (III No 3). There is also information here (dated 23 VIII 1934) that the plaque lacks a coat of arms, catacombs A, row III-3.
