Burial place marked by a statue of Immaculata on a two-tier plinth and two-tier base. Statue on its own base. Mother of God depicted frontally wearing a long robe tied at the waist and cloak, fastened at the neck with a fibula. Her hands are lowered, facing the worshippers. Long hair falls over her shoulders and down her back, with a crown on her head. Mary, standing on a globe, tramples a serpent with her bare feet. First step of plinth with chamfered top edges. Engraved inscription to the front. The second step of the pedestal covered with a profiled hipped roof. On the front a panel closed with an arch, in which a relief of St. Francis of Assisi is carved. The Saint is portrayed in frontal view, wearing a habit, with a rosary hanging from his waist. His left hand touches the cross, while in his right he holds a closed book. To the left lies a skull. Second step of base with chamfered upper edges.

Inwentaryzacja cmentarzy dawnego powiatu zbaraskiego rozpoczęta została w roku 2019 przez zespół z Instytutu Historii Sztuki Uniwersytetu Kardynała Stefana Wyszyńskiego w Warszawie pod kierunkiem prof. Anny Sylwii Czyż i dr. Bartłomieja Gutowskiego. Ze względu na pandemię COViD, a następnie działania wojenne prace od roku 2020 zostały wstrzymane. Materiał opracowany został w formie kart ewidencyjnych obiektów zabytkowych i w formie kart inwentaryzacyjnych w katalogu on-line - http://cmentarzetarnopolskie.uksw.edu.pl/. Dokumentacja obejmuje wszystkie nagrobki związane z polskim dziedzictwem kulturowym (przede wszystkim rozstrzygający jest napis w języku polskim) powstałe przed 1945 rokiem.
