Headstone marked by a statue of St James the Elder on a two-stage plinth and base. The figure on its own base. The saint holds a palm branch in his right hand and supports his left hand on a stick. Dressed in a long undulating robe. Long hair, beard. Bare feet visible. The second step of the plinth covered by a wider panel. To the front in a panel enclosed by an overhanging arch a full relief carved figure of the Virgin and Child. Mary in slight contrapposto. Dressed in long ruffled robe and mantle, veil on head. Shoes visible. The Child holds her right hand in a gesture of blessing.

Inscription:

DOM WIECZNA. TU / SPOCZYWA. S. P. / JAKUB. I PAWLINA / SZUBER. ON PRZeŻYŁ / LAT 54 UMARł DNIA [16] / PAŹDZIERNIKA ROKU / 1900 //.
