Jan Maczek's grave is located in the Antokol cemetery in the Polish quarters of soldiers killed in the war against the Bolsheviks. The discovery of the gravestone was made by Yvette Poplawska, president of the International Association of Artists Authors Journalists Virtualia ART, based in Belgium.

The burial site is an earthen grave marked by a concrete cross. The cross is straight, on a plinth. Between the arms of the cross a gloria of short, truncated rays. At the intersection of the arms an engraved inscription: S. † P. / MACZEK JAN / PPOR. 38 P. STRZ / 1920 / 5. 28. //.
