Burial site marked by a cross on a two-stage plinth and two-stage base. Cross on plinth with chamfered side and top edges. At the intersection of the arms of the cross a figure of Jesus crucified in relief. Above a titulus. Below a simplified skull. The first step of the plinth with chamfered upper edges. On the right side an engraved inscription (1), on the front two laurel wreaths engraved. The second step of the plinth covered with a simplified beam and a hipped roof. To the front, a relief figure of an unknown saint in a panel. The figure stands frontally, right hand resting on the chest, left hand holding a cross. The figure has long hair and is dressed in episcopal garb and headdress. Below the figure an engraved inscription (2).

Inwentaryzacja cmentarzy dawnego powiatu buczackiego wykonana została w latach 2017-2019 przez zespół z Instytutu Historii Sztuki Uniwersytetu Kardynała Stefana Wyszyńskiego w Warszawie pod kierunkiem prof. Anny Sylwii Czyż i dr. Bartłomieja Gutowskiego. Materiał opracowany został w formie kart ewidencyjnych obiektów zabytkowych i opublikowany w książce "Cmentarze dawnego powiatu tarnopolskiego", t. 1 i 2, pod red. A. S. Czyż i B. Gutowskiego, Warszawa 2017-2019 roku. Karty inwentaryzacyjne opublikowane zostały w katalogu on-line - http://cmentarzetarnopolskie.uksw.edu.pl/. Dokumentacja obejmuje wszystkie nagrobki związane z polskim dziedzictwem kulturowym (przede wszystkim rozstrzygający jest napis w języku polskim) powstałe przed 1945 rokiem.
