Burial place originally marked by a cross on a three-tier plinth and base, in a band. Cross not preserved, on high moulded plinth. Base and first step of plinth with chamfered upper edges. Second step of plinth with chamfered upper edges. In front a panel with segmental arch carved corners, in which a relief inscription is carved. The third step of the pedestal tapering upwards, with chamfered vertical edges, covered with a moulded panel with acroterions and triangular reliefs to the front and rear. To the front, a panel with carved segmental arch corners, closed with a gable. In it bas-relief: book, stole, chalice with host.

In the file of Wacław Wejtka, under the letter J, card number 221, dated 1936. According to the file, the old location of the gravestone is sector "C, 3rd row, N12. From the road towards catacombs A." In the description: "Dark polished granite monument with cross. (...) Iron lattice fence".

State of preservation:

mosses, lichens, non-maintained finial
