Osowce | Ukraine
Burial place marked by cross on plinth and base. Cross with arms ending in a stylised trillium form. The plinth slightly tapering towards the top with profiled upper edges. In front a panel with quarter round chamfered upper corners. In it, an engraved inscription.

Inscription:

+ / TU / SPOCZYWA / JAN / KOZUBOWSKI / * 2.III.1854 / + 23.IX.1930 / FUNDATOR / ANTONINA I JULIAN / KOZUBOWSKI //.
