Headstone marked by a gravestone, at the head of which is a cross on a two-stepped plinth and two-stepped base. Cross with moulded plinth. First step of plinth with chamfered upper edges. The second step of the plinth with a wider panel on top, the front with an engraved inscription. To the left in a niche a carved figure of St Joseph, wearing a long undulating robe, his hair and beard are short. He holds a book in his hands. On the right in the niche, a carved figure of St Martin wearing a knee-length robe and clasped cloak, a helmet on his head. He cuts off half of his cloak with a sword. In kneeling position, a male figure, his right hand grasps the saint's robe, in his left hand a stick.

Inwentaryzacja cmentarzy dawnego powiatu zbaraskiego rozpoczęta została w roku 2019 przez zespół z Instytutu Historii Sztuki Uniwersytetu Kardynała Stefana Wyszyńskiego w Warszawie pod kierunkiem prof. Anny Sylwii Czyż i dr. Bartłomieja Gutowskiego. Ze względu na pandemię COViD, a następnie działania wojenne prace od roku 2020 zostały wstrzymane. Materiał opracowany został w formie kart ewidencyjnych obiektów zabytkowych i w formie kart inwentaryzacyjnych w katalogu on-line - http://cmentarzetarnopolskie.uksw.edu.pl/. Dokumentacja obejmuje wszystkie nagrobki związane z polskim dziedzictwem kulturowym (przede wszystkim rozstrzygający jest napis w języku polskim) powstałe przed 1945 rokiem.
