Burial site originally marked by a cross on a two-stage plinth and two-stage base. Cross damaged. On its own plinth with chamfered top edges. Visible leg of Christ and piece of torso. The rest not preserved. First step of plinth with moulded top edges. On the front an engraved frame with quarter chamfered corners with an engraved inscription. The second step of the plinth surmounted by a Corinthian stylised capitol. The second step of the base moulded, in the form of a hipped canopy.

Inscription:

PO S P KAROLU. / KRZYŻANOWSKIM / 1887 + ZA * / STARANIEM ANNY / [..]K[..]OKA[.] //.
