Burial place marked by cross on plinth, in front of which two bands in the form of coffin lid on base. Cross on moulded plinth. Plinth with 1/3 height profiled sides and chamfered upper edges. At the front on the plinth a plaque with chamfered upper corners with an engraved inscription.

Ś. + P. / LESZEK / JEZIERSKI / zm. 6 V 1944 R. / REGINA / JEZIERSKA / zm. 23 V 1945 R. / Przeżywszy 5 TYGODNI / spij spokojnie DROGIE DZIECIE //.
