Burial site originally marked by a cross on a plinth in the form of a tree on the rocks. The cross is not preserved. On the plinth on the front an oval panel after a photograph, below a plaque in the form of an open book with panels with quarter-circular chamfered corners, in which is a relief inscription (1). Below, on the rocks to the front, an irregular plaque with a relief inscription (2). To the right of the plaque a relief inscription. The whole together with tombstones numbers 002, 003 and 004 enclosed by a cast iron fence with a gate at the front left.

1. S. P. / tu spo- / czywa / MARYA / z Żelskich / KAMINSKA / zm. / 9 kwiet. / 1904 r. / wieku / lat 32. / zbaw / je panie! //.
2. Dzień on / dzień zejścia mojego / W-dniu imienin i / powinszowania / Pożegnałam rodzine i / małżąka miłego / Aż do dnia zmartwych / powstania! //.
