Tombstone originally marked by a cross on a two-stage plinth and base. Cross not preserved. Base with chamfered upper edges. First step of plinth with moulded upper edges. On the front an engraved inscription (2). The second step of the pedestal with chamfered upper edges. On the front, engraved inscription (1).

Inscription:

1. Tu / spoczywają zwłoki / S. P. / Michaliny / TORWERT / ZM. 1 Nowembra / 1875 R. / wieku lat 37. / -- / Wieczne odpocznienie / racz Jej dać Panie. //.
2. Dla najlepszej Żony i / Matki mąż z dziećmi / rę pamiątkę poświęcają //.
