Rectangular ground tomb, covered with a glazed slab, at the head of which is a cross stylised as a tree trunk on a rock-shaped pedestal and base. At the intersection of the arms is a relief carved crown of thorns and the inscription IHS. At the front of the tomb, on the axis, the entrance opening, framed by bonia, closed with a segmented arch, covered by a panel with a rhytic inscription.

Inwentaryzacja cmentarzy dawnego powiatu buczackiego wykonana została w latach 2017-2019 przez zespół z Instytutu Historii Sztuki Uniwersytetu Kardynała Stefana Wyszyńskiego w Warszawie pod kierunkiem prof. Anny Sylwii Czyż i dr. Bartłomieja Gutowskiego. Materiał opracowany został w formie kart ewidencyjnych obiektów zabytkowych i opublikowany w książce "Cmentarze dawnego powiatu tarnopolskiego", t. 1 i 2, pod red. A. S. Czyż i B. Gutowskiego, Warszawa 2017-2019 roku. Karty inwentaryzacyjne opublikowane zostały w katalogu on-line - http://cmentarzetarnopolskie.uksw.edu.pl/. Dokumentacja obejmuje wszystkie nagrobki związane z polskim dziedzictwem kulturowym (przede wszystkim rozstrzygający jest napis w języku polskim) powstałe przed 1945 rokiem.
