Burial site marked by unidentified figure on plinth and two-stage base. Figure on its own base, with head lost. Shown frontally, she presses a crucifix to her breast with both hands. Dressed in a long robe and cloak, feet barefoot.
First step of plinth with moulded upper edges. Second step of plinth with simplified beam, comprising architrave and cornice, covered by hipped roof. On the plinth, engraved frames on three sides enclosed at the top by a full overhanging arch. On the front an engraved inscription.

S + P / ROZALIJA / BŁASZCZYSZYN / ŻYŁA LAT 70 / UMARŁA W RO / 190[9] //.

Inwentaryzacja cmentarzy dawnego powiatu buczackiego wykonana została w latach 2021-2022 9 przez zespół specjalistów z Instytutu Historii Sztuki Uniwersytetu Kardynała Stefana Wyszyńskiego w Warszawie i Muzeum Narodowego w Warszawie w składzie dr Paweł Drabarczyk vel Grabarczyk, dr. Bartłomiej Gutowski (kierownik projektu), dr Magdalena Olszewska dr Magdalena Tarnowska. Materiał opracowany został w formie kart ewidencyjnych obiektów zabytkowych i opublikowany w katalogu Instyttutu Polonika. Dokumentacja obejmuje wszystkie nagrobki związane z polskim dziedzictwem kulturowym (przede wszystkim rozstrzygający jest napis w języku polskim) powstałe przed 1945 rokiem.
