Burial place marked by a two-stage base and a two-stage plinth without surviving finial of the first stage of the plinth from the moulded upper edges. An engraved inscription to the front. The second step of the pedestal finished with a simplified beam and covered with a profiled panel, on the front a panel ending in a semicircle, inside it a relief St. John (martyr).

Inwentaryzacja cmentarzy dawnego powiatu zbaraskiego rozpoczęta została w roku 2019 przez zespół z Instytutu Historii Sztuki Uniwersytetu Kardynała Stefana Wyszyńskiego w Warszawie pod kierunkiem prof. Anny Sylwii Czyż i dr. Bartłomieja Gutowskiego. Ze względu na pandemię COViD, a następnie działania wojenne prace od roku 2020 zostały wstrzymane. Materiał opracowany został w formie kart ewidencyjnych obiektów zabytkowych i w formie kart inwentaryzacyjnych w katalogu on-line - http://cmentarzetarnopolskie.uksw.edu.pl/. Dokumentacja obejmuje wszystkie nagrobki związane z polskim dziedzictwem kulturowym (przede wszystkim rozstrzygający jest napis w języku polskim) powstałe przed 1945 rokiem.
