Burial site originally marked by cross on plinth and base. Cross stylised on tree trunk, forfeited, fragments lie beside. Plinth stylised as a pile of stones. On the front a relief carved plaque in the form of an unrolled scroll, on which is an inscription engraved.

Inscription:

STANISŁAW / ALBIK JAROSZ / URZĘDNIK / WYDZIAŁU POW. / * 5/2 1908 + 1/8 1930 / PROSI O ANIOŁ PAŃSKI. //.
