Tombstone marked by a cross on its own moulded plinth and two-stepped plinth with a tombstone added to the front. The arms of the cross profiled, ending in a semicircle, framed by shafts. At the intersection of the cross arms a relief medallion within which is a bust of a saint with long hair. The face is illegible. The first step of the pedestal with chamfered top edges, the second step of the pedestal surmounted by moulding. Engraved inscription to the front. Gravestone with an engraved cross on the face.

Inwentaryzacja cmentarzy dawnego powiatu zbaraskiego rozpoczęta została w roku 2019 przez zespół z Instytutu Historii Sztuki Uniwersytetu Kardynała Stefana Wyszyńskiego w Warszawie pod kierunkiem prof. Anny Sylwii Czyż i dr. Bartłomieja Gutowskiego. Ze względu na pandemię COViD, a następnie działania wojenne prace od roku 2020 zostały wstrzymane. Materiał opracowany został w formie kart ewidencyjnych obiektów zabytkowych i w formie kart inwentaryzacyjnych w katalogu on-line - http://cmentarzetarnopolskie.uksw.edu.pl/. Dokumentacja obejmuje wszystkie nagrobki związane z polskim dziedzictwem kulturowym (przede wszystkim rozstrzygający jest napis w języku polskim) powstałe przed 1945 rokiem.
