Tombstone originally marked by a cross on a plinth banded in the form of a coffin lid. Cross broken, not preserved. The plinth tapering upwards, with chamfered edges to the front. On it on the front an engraved inscription.

Inscription:

S. + P. / WŁADYSŁAW / KOSSAKOWSKI / WICE PREZES / ZARZONDU OKRE / Z.Z.K. / LAT 62 + 5/VII 1924 / WIECZNA PAMIĘĆ //.
