Burial place marked by a headstone with a cross on a two-stepped plinth and base. Gabled slab. Cross on plinth. Cross arms chamfered, inside a repeated engraved cross shape. Base with chamfered upper edges. The first step of the pedestal with profiled upper edges. the second step of the pedestal enclosed by a projecting segmental arch. On the pedestal a chapel with a gabled roof on columns, on an extended base. In the field of the chapel on the second step of the pedestal an engraved inscription.

Inscription:

DOM / D WŁADYSŁAW / WEGRZYNOWSKI / LEKARZ POWIATOWY / 8 wrzesnia 1857 / + 2 maja 1895 / Prosi o westchnienie do Boga / WŁADKU! DOWIDZENIA / W NIEBIE //.

Inwentaryzacja cmentarzy dawnego powiatu zbaraskiego rozpoczęta została w roku 2019 przez zespół z Instytutu Historii Sztuki Uniwersytetu Kardynała Stefana Wyszyńskiego w Warszawie pod kierunkiem prof. Anny Sylwii Czyż i dr. Bartłomieja Gutowskiego. Ze względu na pandemię COViD, a następnie działania wojenne prace od roku 2020 zostały wstrzymane. Materiał opracowany został w formie kart ewidencyjnych obiektów zabytkowych i w formie kart inwentaryzacyjnych w katalogu on-line - http://cmentarzetarnopolskie.uksw.edu.pl/. Dokumentacja obejmuje wszystkie nagrobki związane z polskim dziedzictwem kulturowym (przede wszystkim rozstrzygający jest napis w języku polskim) powstałe przed 1945 rokiem.
