Headstone marked with a statue of the Virgin Mary on a two-stage plinth and two-stage base. The figure on its own base. Mary holds her hands in a prayer gesture. Dressed in long ruffled robe and mantle, veil on head. First step of plinth with chamfered upper edges. Second step of plinth with simplified beam consisting of architrave and cornice, covered by moulded wider panel. On the front an engraved inscription.

Inscription:

ZOFIA / SMOLUK / żyła lat 71 / + 1912 / Prosi o Anioł Pański //.

Inwentaryzacja cmentarzy dawnego powiatu zbaraskiego rozpoczęta została w roku 2019 przez zespół z Instytutu Historii Sztuki Uniwersytetu Kardynała Stefana Wyszyńskiego w Warszawie pod kierunkiem prof. Anny Sylwii Czyż i dr. Bartłomieja Gutowskiego. Ze względu na pandemię COViD, a następnie działania wojenne prace od roku 2020 zostały wstrzymane. Materiał opracowany został w formie kart ewidencyjnych obiektów zabytkowych i w formie kart inwentaryzacyjnych w katalogu on-line - http://cmentarzetarnopolskie.uksw.edu.pl/. Dokumentacja obejmuje wszystkie nagrobki związane z polskim dziedzictwem kulturowym (przede wszystkim rozstrzygający jest napis w języku polskim) powstałe przed 1945 rokiem.
