The burial site marked by a cross stylised as a tree trunk on a two-stage plinth in the form of stone blocks and a two-stage base. Cross decorated with relief carved oak branches. At the intersection of the arms a relief carved figure of Christ, above a titulus. The steps of the base glazed. On the first step of the pedestal at the front a plaque in the form of an unfolded scroll with an engraved inscription. On the second step of the pedestal on the front a relief of St. Anthony. The saint is dressed in a habit girt at the hips. In his right hand he holds a lily, in his left hand a book and a rosary.

Inwentaryzacja cmentarzy dawnego powiatu buczackiego wykonana została w latach 2005-2007 przez zespół z Instytutu Historii Sztuki Uniwersytetu Kardynała Stefana Wyszyńskiego w Warszawie pod kierunkiem dr Anny Sylwii Czyż i dr. Bartłomieja Gutowskiego. Materiał opracowany został w formie kart ewidencyjnych obiektów zabytkowych i opublikowany w książce "Cmentarze dawnego powiatu buczackiego", pod red. A. S. Czyż i B. Gutowskiego, Warszawa 2017 roku. Dokumentacja obejmuje wszystkie nagrobki związane z polskim dziedzictwem kulturowym (przede wszystkim rozstrzygający jest napis w języku polskim) powstałe przed 1945 rokiem. Karty inwentaryzacyjne przechowywane są w Warszawie przez Archiwum Akt Nowych (zespół dokumentów Stowarzyszenia „Wspólnota Polska”) oraz Departament Dziedzictwa Kulturowego Ministerstwa Kultury i Dziedzictwa Narodowego.
