Miejsce pochówku oznaczone pierwotnie krzyżem na cokole w formie stosu kamieni. Krzyż utrącony, leży obok. Cokół przewrócony. Od frontu tablica z uszakami obwiedziona wypukłorzeźbioną, profilowaną ramką, gdzie inskrypcja ryta. Nagrobek leży w grupie destruktów.
