On a sloping pedestal, a stepped light-weight flat panel, at the head a square stele on which a cross in black marble is superimposed on the left, in the upper left part a photograph on porcelain is fixed.

Inscriptions: on the stele (1), on a metal plaque superimposed on a red marble plaque, standing on the slab (2) and on a stone plaque standing on the slab to which a metal rose branch is superimposed (3).

Inscription:

2. Elżbieta BERNARDIN / née KUCZEK / 1954 - 1989 //.
