On a low pedestal a flat slab behind which, at the headrest, is another smaller and thinner slab on which now lies an overturned cross faced with panels, at the crossing of the arms a five-pointed star, traces of a fence on the pedestal, in the middle of the marble slab an engraved small cross. Inscriptions: on the small slab lying at the headboard (1) and on the slab (2).

Comments on the state of preservation and visible restoration:

After reconstruction (by the efforts of the TOZ with funds from the Zygmunt Zaleski Foundation (ATOZ)) in 2008, the original condition was restored: a flat panel on the band, a low wide pedestal at the headrest on which a cross on a base of volutes with thistle flowers in relief was placed.
