On the pedestal is a stepped thin gabled panel, at the head on a two-sided pedestal is a tall cross supported by volutes with a faceted panel on the arms, the arms ending in poppy(?) flowers.

Comments on the state of preservation and visible restoration:

Originally surrounded by a high cast-iron fence according to lithography by S. Karnicka from 1850; in 1986 the gravestone was damaged, elements scattered around the grave site; 2008 the gravestone was restored by TOZ with funds from the Zygmunt Zaleski Foundation (ATOZ); survey drawings, 3 shots, scale 1:20 by B. Kłosowicz [2008](ATOZ); Documentation of conservation works carried out at the Père-Lachaise cemetery in Paris, Grave of Franciszka Czacka née Jełowicka, commissioned by the Society for the Care of Polish Historical Monuments and Tombs in France, bd.
