Burial place originally marked by cross on plinth and base. Cross not preserved. Base with chamfered upper edges. Plinth with moulded upper edges. Engraved inscriptions to the front (1), (4), left (2) and rear (3).

1) Tu spoczywają zwłoki / Ś. P. / WINCENTEGO / CZYŻA / Kapitana / um. 10 Paździer. 1881 r. //.

2) Ś. P. / HELENA / z Giedrojciów / CZYŻOWA / Prosi o trzy zdrowaś / Marya. //. 3) Ich chrzestna córka / EMILIJA / KOZŁOWSKA //.

According to Wacław Wejtko's file, the gravestone was originally surmounted by a cross and enclosed by a fence with iron bars. Wejtko gives the inscription: Here lie the remains of / Ś. P. / Wincentego / Czyża / Kapitan / Um. 10 Oct. 1881 / God save his soul //.
