In January/February 1942, the redeployment of the Polish Armed Forces in the USSR (General Anders' Army) from the Soviet Russian republics to the Central Asian republics took place. Units of the 6th Infantry Division and the Army Organisation Centre were located in the then Kashkadarya region. There was also a massive influx of civilians to the places where Polish units were being formed, following the decree of the Presidium of the Supreme Soviet of the USSR on 12.08.1941 on the so-called "amnesty for Polish citizens."

Soldiers of various units of the 6th DP who died in the garrison hospital located in this town, as well as civilians, were laid to rest in the cemetery in Shakhrisabz. During the Soviet era the traces of the cemetery were gradually erased and it was decided to build a school on the site. The remains of the Poles were then moved to another site - on a hill in the city centre. A simple memorial was made using a tall gravestone that had been preserved since the war, under which a large marble slab was placed. The monument was surrounded by a chain-link fence suspended from steel posts. The inscription on the gravestone reads: "WINA [Nina] / CHĄDZYŃSKA-DAGE / ADAM'S LOVED HEART / DIED 3 APRIL 1942 / AND HERE SHE REMAINS / OCEAN EARTH / HARD WAS HER FOR LIFE / BE HER LIGHT IN DEATH / MY SPIRIT WILL NEVER FORSAKE YOU / CMW DOK". A boulder with another surviving inscription was also placed nearby: "YOUR CORPSE / IN A FOREIGN LAND / BUT YOUR SPIRIT IN POLAND / AMONG YOUR OWN / DIED IN SERVICE / FOR POLAND / 6 DP". When a delegation from the Council for the Protection of Struggle and Martyrdom Remembrance first arrived here in the 1990s, clear traces of individual earthen graves were visible behind the monument. In 1998, the local authorities cleaned up the site - it was fenced off from the street with an ornamental stone fence with a metal gate, and concrete steps led to the monument.

The cemetery in Shakhrisabz was rebuilt by the OPWiM Council in 2001. The surviving gravestone monument was moved to a new location on the axis of the entrance in the depth of the cemetery, and a milepost with the characteristic rectangular shape, uniform for all rebuilt cemeteries in Central Asia, topped with an eagle bas-relief was erected as the central monument. Its form refers to the soldier's wandering. The monument bears an inscription in Polish and Uzbek: "TU SPOCZYWAJĄ POLACY / 256 SOLDIERS / OF THE POLISH ARMY / IN THE EAST / GEN. WŁ. ANDERS / AND CIVILIANS / FORMER PRISONERS OF WAR / PRISONERS / OF SOVIET CAMPS / WHO DIED IN 1942. / ON THEIR WAY TO THEIR HOMELAND / HONOUR THEIR MEMORY".

On the symbolic rows of graves created behind the monument, tombstones with the relief of a cross were laid. Directly behind the monument are plaques with the names of the soldiers buried here, established in 2001. The cemetery has been fenced off with a low stone wall.

After the liquidation of the OPWiM Council, the care of the cemetery is financed by the Ministry of Culture and National Heritage. The Ministry of Culture and National Heritage is also conducting research work to establish a census of civilians buried in cemeteries in Central Asia and to verify the names of soldiers.

In 2017, the information plaques at the entrance to the cemetery were changed (one in Polish and the other in Uzbek). The Polish content is as follows: "POLISH / WAR CEMETERY / SHAKHRISABZ / UNDER THE CARE / OF THE / MINISTRY OF CULTURE AND HERITAGE / NATIONAL HERITAGE / RECONSTRUCTED IN 2001 / THROUGH THE EFFORTS OF THE COUNCIL FOR THE PROTECTION OF THE MEMORY / OF BATTLES AND MARTYRDOM".
