Burial site marked by a cross on a two-stepped plinth of a two-stepped base. Cross stylised as an oak trunk with bas-relief leaves, on its own plinth with chamfered and bevelled corners. At the intersection of the arms a convex carved medallion with a convex carved bust of Christ surrounded by engraved rays. Christ facing right, wearing crown of thorns. The first step of the pedestal with chamfered top edge, with engraved inscription to the front (1). Second step of plinth surmounted by simplified beam with stylised volutes to front. On the front a semi-circular niche with a bas-relief figure of St. Thomas framed frontally, with a spear in his left hand and his right hand on his heart. Below the niche an engraved inscription (2). To the right and left analogous niches with relief figures. On the right, St. Elizabeth portrayed frontally with a palm leaf in her left hand, below which is an engraved inscription (3). To the left, St. Angelus portrayed frontally, with a cross applied to her heart, supported by her right hand, below which is an engraved inscription (4).

Inwentaryzacja cmentarzy dawnego powiatu zbaraskiego rozpoczęta została w roku 2019 przez zespół z Instytutu Historii Sztuki Uniwersytetu Kardynała Stefana Wyszyńskiego w Warszawie pod kierunkiem prof. Anny Sylwii Czyż i dr. Bartłomieja Gutowskiego. Ze względu na pandemię COViD, a następnie działania wojenne prace od roku 2020 zostały wstrzymane. Materiał opracowany został w formie kart ewidencyjnych obiektów zabytkowych i w formie kart inwentaryzacyjnych w katalogu on-line - http://cmentarzetarnopolskie.uksw.edu.pl/. Dokumentacja obejmuje wszystkie nagrobki związane z polskim dziedzictwem kulturowym (przede wszystkim rozstrzygający jest napis w języku polskim) powstałe przed 1945 rokiem.
