Tombstone marked with cross on three-stage plinth and base. Cross on moulded plinth. First step of plinth with chamfered upper edges. The second step of the plinth covered by a wider moulded panel. On the front in an engraved frame with chamfered quarter-circle corners an engraved inscription. The third step of the pedestal in the form of an aedicula, closed at the front with an irregular arch, with acroterions at the corners. In a niche enclosed by a solid arch, a figural group of the Holy Family, on its own base. On the left is a figure of Mary. She holds her right hand lowered, her left hand at heart level. She is dressed in a long ruffled robe, girdled at the waist, a veil on her head. Long hair. On the right is the figure of St Joseph. The Saint holds a palm branch in his left hand. Dressed in long undulating robe. Long hair, beard. In front of them a figure of the young Christ. He holds his right hand at heart level, left hand lowered. Dressed in long corrugated robe. Bare feet visible.

Inwentaryzacja cmentarzy dawnego powiatu zbaraskiego rozpoczęta została w roku 2019 przez zespół z Instytutu Historii Sztuki Uniwersytetu Kardynała Stefana Wyszyńskiego w Warszawie pod kierunkiem prof. Anny Sylwii Czyż i dr. Bartłomieja Gutowskiego. Ze względu na pandemię COViD, a następnie działania wojenne prace od roku 2020 zostały wstrzymane. Materiał opracowany został w formie kart ewidencyjnych obiektów zabytkowych i w formie kart inwentaryzacyjnych w katalogu on-line - http://cmentarzetarnopolskie.uksw.edu.pl/. Dokumentacja obejmuje wszystkie nagrobki związane z polskim dziedzictwem kulturowym (przede wszystkim rozstrzygający jest napis w języku polskim) powstałe przed 1945 rokiem.
