Burial site originally marked by cross on plinth and base. Preserved plinth of cross, with chamfered upper edges. Base with moulded edges. Two-stage plinth, first stage with chamfered upper edges. On the front a rectangular panel with chamfered quarter circular upper corners, with a relief inscription. The second step of the plinth closed with a gable. On the front a panel with quarter-circular chamfered lower corners, with a bas-relief inscription. On the left and right sides, panels enclosed by a segmental arch, with chamfered quarter-circle lower corners, with relief inscriptions.

1. Ś. † P. / GROBY RODZINNE MIŃKOWSKICH / OLIMPJA MARJA / ZASN. W BOGU 27 / WRZEŚNIA 1916 R. / MAJĄC LAT 22 / CZĘŚĆ JEJ PAMIĘCI. //.
2. I usłyszałem głos z nieba / mówiący mi: napisz: / Błogosławieni umarli, / Którzy umierają w Panu / Obi XIV 8 // Prawy bok: Ś. † P. / JÓZEFA Z SZYŁEJKÓW / MIŃKOWSKA / UR. 1866 R. / ZM. / BOŻE ZBAW / JEJ DUSZĘ //.
3. Ś. † P. / KAZIMIERZ / MIŃKOWSKI / UR. 1863 R. ZM. / POKÓJ JEGO DUSZY //.

The dates of death on the right and left sides have not been uncovered. According to Wacław Wejtka's file, the gravestone was originally framed by a fence of "iron bars"
