Headstone marked with a figure of Archangel Michael on a two-stepped plinth and three-stepped base. The figure on its own base. Archangel dressed in antique armour and sandals, in his left hand he holds a small oval shield on which is carved the inscription IHS. The archangel stands on a recumbent figure of Satan, depicted as an elderly man with wings, long hair combed back and a beard. who holds a pitchfork in his right hand. The first step of the plinth with moulded upper edges. On the front (1), on the right (2) and left (3) sides an engraved inscription. Second step of plinth high with simplified beam, covered by wider panel. On the sides engraved frames with quarter-circle chamfered corners, in the right side an engraved inscription (4). On the front a panel closed with an overhanging arch, in which is a bas-relief figure of St Teresa, below which is an engraved inscription (5). The Saint is depicted wearing a habit tied at the waist with a rosary, her left hand showing the inside of her palm, her right hand bent at the elbow, her hand holding a cross on her chest. In the lower left corner of the panel a bas-relief vase with flowers.

Inwentaryzacja cmentarzy dawnego powiatu zbaraskiego rozpoczęta została w roku 2019 przez zespół z Instytutu Historii Sztuki Uniwersytetu Kardynała Stefana Wyszyńskiego w Warszawie pod kierunkiem prof. Anny Sylwii Czyż i dr. Bartłomieja Gutowskiego. Ze względu na pandemię COViD, a następnie działania wojenne prace od roku 2020 zostały wstrzymane. Materiał opracowany został w formie kart ewidencyjnych obiektów zabytkowych i w formie kart inwentaryzacyjnych w katalogu on-line - http://cmentarzetarnopolskie.uksw.edu.pl/. Dokumentacja obejmuje wszystkie nagrobki związane z polskim dziedzictwem kulturowym (przede wszystkim rozstrzygający jest napis w języku polskim) powstałe przed 1945 rokiem.
