Gravestone marked by a statue of the Virgin Mary on its own plinth, two-stage base and two-stage plinth. Object disjointed, the figure and the slab covering the plinth lie adjacent to the monument. The statue of a full figure with a decapitated head in long robes. In her arms at her chest she holds a child with severed limbs and head. First step of plinth finished with chamfered top edges. The second step of the plinth originally finished with a simplified beam covered by a larger panel. On the front an engraved inscription.

Inwentaryzacja cmentarzy dawnego powiatu zbaraskiego rozpoczęta została w roku 2019 przez zespół z Instytutu Historii Sztuki Uniwersytetu Kardynała Stefana Wyszyńskiego w Warszawie pod kierunkiem prof. Anny Sylwii Czyż i dr. Bartłomieja Gutowskiego. Ze względu na pandemię COViD, a następnie działania wojenne prace od roku 2020 zostały wstrzymane. Materiał opracowany został w formie kart ewidencyjnych obiektów zabytkowych i w formie kart inwentaryzacyjnych w katalogu on-line - http://cmentarzetarnopolskie.uksw.edu.pl/. Dokumentacja obejmuje wszystkie nagrobki związane z polskim dziedzictwem kulturowym (przede wszystkim rozstrzygający jest napis w języku polskim) powstałe przed 1945 rokiem.
